Rehabilitation

With dedicated experience and training in senior care, our licensed therapists work diligently to help our participants get back to their functional baseline. Our compassionate clinicians are a critical piece of the ElderONE interdisciplinary team – serving our participants both at the PACE Center and in the comfort of the participant’s home.

Each participant’s rehabilitation progress is carefully monitored to meet the goals set in their individualized care plan. Some participants may wish to maintain their strength while others may be on the mend, looking to regain as much of their strength and mobility as possible so they can continue to do the things that they enjoy.

ElderONE’s comprehensive rehabilitation services include:

• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Respiratory Therapy
• Medical Equipment (walkers, canes, wheelchairs, as medically necessary)

ElderONE participants receive annual evaluations by physical and occupational therapists, or more often as determined by their individual care plan.  Ongoing therapy helps ElderONE participants with activities of daily independent living so they remain independent and enjoy the highest quality of life for as long as possible.
