Personalized Care That Revolves Around You and Your Family
ElderONE, an affiliate of Rochester Regional Health, is a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement.
As the area’s only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.
A dedicated, interdisciplinary team of geriatric healthcare specialists work with each participant, and their family, to create an individualized health care plan catered specifically to their needs while allowing them to remain independent at home for as long as possible.
To preserve and sustain the independence of chronically ill or disabled individuals by providing comprehensive medical care and support services that allow them to enjoy the highest quality of life in their own home and community.
