Our Care Team

When caring for our participants in the community, ElderONE operates as an 11-person interdisciplinary team – all of whom are specially trained in the aging process. Working in synergy, our multifaceted team of dedicated health care professionals help each participant through the continuum of care, keeping our participant’s safety and wellbeing at the center of all we do.

Our ElderONE care team assesses the needs of every participant - working alongside them and their families to develop an individualized care plan. As care needs evolve, the care team delivers and monitors all services, and adjusts the care plan as needed. For the convenience of the participant, care can be provided in a variety of settings— in the home, at an ElderONE PACE day center, and in a nursing home or a hospital.

ElderONE’s interdisciplinary team meets regularly with participants to ensure that their comprehensive medical and social needs are not only being met but also adjusted after any change in status. Behind the scenes, our care teams meet daily to discuss any changes to their medical, functional, and psychosocial status – helping our participants stay out of the hospital.

    Stephen D. Ryan, MD, MPH, AGSF, FACP - Medical Director of ElderONE

    Dr. Ryan earned his medical degree at the University of Buffalo. He completed his residency in Internal Medicine at Johns Hopkins Bayview Medical Center, where he also completed a Fellowship in Geriatrics and Primary Care Research. Dr. Ryan is Board-certified in Internal Medicine, Geriatric Medicine and Hospice & Palliative Care. Additionally, he serves as a Senior Medical Director for Geriatric Medicine, Rochester Regional Health, and Clinical Assistant Professor of Medicine at the University of Rochester School of Medicine and Clinical Instructor at the Rochester Institute of Technology.

    Meredith Cricco, MD - North Park PACE Center

    Dr. Cricco studied medicine at the University of Iceland. She completed her internal medicine residency at Strong Memorial Hospital at the University of Rochester and a fellowship in geriatric medicine at Monroe Community Hospital. Following that she spent eight years caring for elders living in the community and nursing home settings in Iceland, before returning to Rochester in July 2016.

    Yelena Khaitin, MD - Emerson PACE Center

    Dr. Khaitin is the third generation of doctors in her family, grateful to work at ElderONE. She completed her medical degree at the Medical Kirov State Academy in Russia. After immigrating to the US she learned the language and after passing board exams completed her Internal Medicine Residency at Coney Island Hospital in Brooklyn, NY where she was chosen as a Chief Resident for 2012. She then finished her Geriatric fellowship at Winthrop University Hospital in Mineola, NY. Currently Dr. Khaitin is enjoying practicing Geriatrics and living in the charming City of Rochester with her family.

  • Alfred Malomo

    Alfred A. Malomo, MD - Hudson PACE Center

    With a passion for caring for the elderly, Dr. Malomo has a wealth of experience in both surgical and medical specialties. He earned his medical degree at the University of Ibadan in Nigeria, where he also completed a residency in Orthopedics. He did a second residency in General Surgery at the Royal College of Surgeons in Ireland and England. He also completed Fellowships in Geriatrics at Oregon Health and Science University and in Internal Medicine at the Harlem Hospital Center at Columbia University. Dr. Malomo is Board-certified in Internal Medicine and Geriatric Medicine.

    Anne Olinger, MD - Alternative care sites, Sisters of Mercy and Sisters of St. Joseph

    Dr. Olinger has long enjoyed working with geriatric patients. She earned her Doctorate in Medicine at Case Western Reserve University, where she participated in the initial programs to encourage students to consider geriatrics as a profession. She completed her residency and geriatric fellowship here in Rochester through the University of Rochester. She is Board certified in both Internal Medicine and Geriatrics. She is married with two children and enjoys calling Rochester her home.
