The majority of comprehensive care services take place at our ElderONE PACE Center locations.
When deciding which ElderONE PACE Center is right for each participant, there are a few factors we take into consideration. First and foremost, we always honor participant preference. If the participant doesn’t have a preference, we factor in their proximity to their home and any specialized needs they may have.
As always, transportation will be coordinated to and from the PACE Center. Each participant’s individualized plan of care will determine the frequency of their visits.
For your convenience and the convenience of caregivers, our ElderONE PACE Centers are open Monday through Friday from 8:30 am to 5:00 pm.
In addition to on-site medical, rehabilitative, and personal care services, ElderONE’s PACE Center offers a variety of enriching activities to aid in the cognitive, functional, and spiritual needs of participants. ElderONE PACE Centers create a setting that promotes personal and social well-being, and provides opportunities for friendships to form.
When participants visit the ElderONE PACE Center, they’re welcome to utilize the following services:
Each month, our recreational therapists develop a calendar of social programs designed to help our participants maintain a balanced social life and enriched quality of life. Activities may include music, games, movies, exercise classes, art classes, computer classes, holiday parties, as well as outside trips to restaurants, shopping centers, festivals, and more!
