Medical Care

One of the premier advantages of ElderONE is our on-site medical care at the PACE Center clinic. With the support of Rochester Regional Health, our extensive provider network allows us to give care to our participants right when they need it. With a primary focus on prevention and early intervention, ElderONE remains committed to caring for participants in a non-institutional setting.

Our comprehensive list of medical care services include the following (as medically necessary):

  • Primary and specialty medical care
    • Cardiology
    • Neurology
    • Optometry
    • Dentistry
  • Acute hospital and long-term care
    • Inpatient hospital services
    • Nursing home care services

  • Emergency and urgent care
  • Prescriptions and over-the-counter medications
  • Durable medical equipment both at home and in rehab (walkers, canes, wheelchairs, hearing aids, eyeglasses, hospital bed, ramps, grab bars, shower chairs etc)
  • Outpatient lab tests and x-rays
  • Medical transportation
Our geriatric physicians, nurses, and nurse practitioners are available from 8:30am to 5:00pm Monday- Friday in all four of our ElderONE PACE Center clinics. In addition, they are on-call 24 hours a day, 7 days a week.

Our ElderONE board-certified geriatric physicians develop, monitor, and supervise each participant’s medical care and treatment plan and work closely with specialty medical physicians in the ElderONE network, including optometry, audiology, dentistry, podiatry, cardiology, pulmonology, rheumatology, among others. Our coordinated care teams meet regularly to review participants’ care plans and make changes when necessary.

If at any time our participants require hospitalization, their care is covered. We will ensure our participants remain comfortable and receive the care they need until they can return home!
